Registering with the surgery
Before you can start accessing services at the surgery, you may be required to complete your registration process with a New Patient Health Check. These appointments are with the practice nurse. The reception staff will make it clear when you speak with them if you fall into this category.
In the remainder of cases, a New Patient Health Check will not be necessary, and your details should be on our system within two to three working days.
Registrations take up to 7 working days to process.
To register with the surgery:
- use the NHS Register with a GP: Cauldwell Medical Centre service
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Record Sharing (Summary Care Records)
As part of your registration, you will be automatically opted in to Summary Care Record sharing. This allows essential information about your health such as medications, allergies, and adverse reactions to be securely shared with other healthcare professionals involved in your care.
If you do not wish to share your Summary Care Record, please contact the practice directly to opt out.

Sharing your health information helps ensure that you receive safe, effective, and timely care — particularly in emergency situations. It enables healthcare professionals to make informed decisions, avoid medication errors, and reduce delays in treatment.
